Annotated Business Associate Agreement [2015]


Under the U.S. Health Insurance Portability and Accountability Act (HIPAA) of 1996, a HIPAA business associate agreement is a contract between a HIPAA covered entity and a HIPAA business associate. The agreement protects personal health information in accordance with HIPAA requirements.

With the support of CSH, Public Counsel’s Community Development Project has designed this form Business Associate Agreement to assist nonprofit supportive housing providers and associated service providers that serve as business associates of covered entities, and the pro bono attorneys who represent them.
